Mosaic Medical Center in St. Joseph, MO is seeking a skilled Advanced Practice Provider for a full-time role in our Continued Care Clinic. The clinic will serve adult patients without a PCP who have been discharged from the hospital or emergency department. The purpose is to provide timely post-hospitalization or post-emergency department follow up, ensure patients continue receiving appropriate care, avoid potential health complications, address health-care barriers, reduce return ED visits and hospitalizations, and transition patients to a primary care provider.

Practice Details:

  • 100% Outpatient
  • Shared call rotation which includes after hour phone calls and addressing patient needs
  • Comprehensive care across all acuity levels
  • Full functioning lab, pharmacy, and outpatient infusion center onsite

NP Requirements:

  • Successful completion of an APRN master’s nursing degree or graduate of accredited PA program
  • Current Missouri license preferred or the ability to obtain licensure
  • Current DEA licensure
  • Primary Care experience preferred but not required

Additional Qualifications:

  • Possess ability to work independently/autonomously and interdependently with other health care professionals
  • Have extensive patient assessment skills using diagnostic methods and instruments with the ability to develop appropriate care plans, modifying as indicated
  • Possess effective communication skills and interpersonal skills with patients, families and members of the health care team

Qualifications

  • Successful completion of a CNS or APRN master’s degree in a nursing program in the specialty area or must demonstrate successful completion of a formal post-graduate track or accredited program (within the specialty area) within a school of nursing granted graduate-level academic credit (e.g. graduate, non-matriculating program)
  • Licensed in state of Missouri or Kansas, depending upon designated work location. Must be currently certified by a nationally recognized certifying body acceptable to the Missouri or Kansas State Board of Nursing as well as hold and maintain a current Document of Recognition
