Job Description
The Medical Social Worker (BSW) provides a professional, trauma informed psychosocial assessment addressing psychological, social, emotional, religious, cultural, financial, and health related needs that impact patient care and discharge planning. The Social Worker utilizes strong communication skills to assess and address barriers, coordinate resources, and support safe transitions of care. This role supports interdisciplinary planning by contributing psychosocial information that informs appropriate level of care and discharge decisions in collaboration with RN Case Management and providers.

The Social Worker utilizes strong communication skills to assess and address barriers, coordinate resources, and support safe transitions of care. This role supports interdisciplinary planning by contributing psychosocial information that informs appropriate level of care and discharge decisions in collaboration with RN Case Management and providers.
Responsibilities
- The Social Worker collaborates with interdisciplinary and post-acute partners to implement and monitor individualized care plans responsive to each patient’s situation.
- The Social Worker engages patients and caregivers in meaningful discussions, reinforces education, and advocates for patient preferences and goals.
- The Social Worker responds to physician and interdisciplinary consultations for identified social needs throughout the facility.
- The Social Worker monitors progress related to psychosocial interventions and communicates changes to the care team.
- Complex clinical, ethical, or safety concerns are escalated in accordance with scope of practice and hospital policy.
- This role supports quality, cost effective outcomes for patients, families, and the organization.
- Active participation in program development, quality improvement, and process improvement initiatives is an expectation of the Medical Social Worker role.
- Other duties as assigned
- Bachelor's Degree - Social Work Degree - Required
- 2 Years - Experience of BSW or two semester practicum training at Mosaic Life Care - Required
- 3 Years - If position is in a Mental\Behavioral Health setting, must have minimum of three years’ experience in similar setting and certification by the National Association of Social Workers. - Required
- Upon Hire
- Must be able to travel between various system facilities and off-site locations as needed. - Required
Skills and Abilities
Essential Technical/Motor Skills
- Hand/eye coordination; speaks clearly; good hearing, ability to use computers, printers and telephone equipment
- Must be able to teach, provide accurate and concise information, answer questions, maintain positive relationships with multiple customers, motivate and persuade others.
- Physical requirements include sufficient strength and mobility to sit for long periods of time
- Assess, comprehend, understand, interpret data to assess patient condition and evaluate care of patient.
- Appropriate functional vision, hearing acuity and smell

- Transportation or driving to and from various locations for the purpose of coordinating member/patient care.
