Summary

  • Develops, plans, directs, coordinates, evaluates and manages activities to promote a quality, evidence based skin and wound care program. Facilitates interdisciplinary collaboration to drive the strategic implementation of skin and wound care best practices, fostering a culture of consistency, continuity of care, and quality of care. Develops and evaluates individualized patient care by applying a clinical knowledge and skill. Oversees the development and delivery of skin and wound care education for patient/family and interdisciplinary staff. Leads program development and initiatives to enhance the quality of skin and wound care. Promotes compliance with regulatory standards by engaging stakeholders to assess the impact of policy changes on patient care delivery and workflow processes. Provides leadership to and oversight of, the wound care nursing staff, and provides direct patient care, education and consultation to providers and staff as needed. Participates in budgetary process. Collaborates with Wound Medical Director. This position is employed by Mosaic Life Care.

Duties

  • Provides direct patient care after analysis of patient status and needs to develop and evaluate individualized care plans by: applying WOC clinical knowledge and skills to analyze and assess patient needs; overseeing collaboratively developed individualized care plans for complex and specialized patients while providing guidance to staff; leading the evaluation of effectiveness of care plans through ongoing assessment, analysis, and revision, anticipating and resolving /mitigating issues and risks as they arise; and providing consultation to patients, families, and interdisciplinary team members to promote patient-centered care, fostering collaboration and communication.
  • Provides oversight and management of wound care staff: conducts recruitment, hiring and onboarding process and daily staffing to meet patient care needs; accountable for performance management of employees including annual performance reviews and follows progressive discipline process for time, attendance and performance deviations; identifies and intervenes with interpersonal or professional conflicts within and with the wound care staff; engages in discussions with wound care staff on their professional development needs including individual growth opportunities as well as mandatory required learning; monitors metrics and takes actions to enhance employee satisfaction; demonstrates ability to delegate tasks using clear communication and monitoring progress.
  • Implements evidence-based practices and clinical guidelines by: conducting literature searches and appraising evidence to maintain currency in specialty area; assessing current clinical practices and outcomes to identify areas for opportunity; strategizing, promoting and leading widespread adoption of best practices in collaboration with interdisciplinary teams.
  • Promotes compliance with regulatory standards and best practices by: engaging stakeholders to assess the impact of policy changes on patient care delivery and workflow processes, fostering collaboration and communication; providing expert input in policy/protocol creation, development, and review processes, ensuring alignment with regulatory standards, evidence-based practice, and organizational goals and objectives; and engaging with staff and stakeholders to evaluate the effectiveness of policies and procedures, resolving issues and making improvements as needed.

Qualifications

  • Bachelor's Degree in Nursing (BSN) required. Graduate of Accredited CWOCN/CWCN Nursing Education Program (wound scope, minimum) required. Master's Degree in Nursing preferred.
  • Registered Nurse (RN) - State Licensure And/Or Compact State Licensure required upon hire. CWCN - Certified Wound Care Nurse required upon hire. CWOCN - Certified Wound Ostomy Continence Nurse required upon hire. CNS - Clinical Nurse Specialist required upon hire.
  • 3 Years of direct patient care in acute care setting required. 1 Year of experience as a Certified Wound Care Nurse in acute care preferred.
