Summary
  • A chaplain at Mosaic Hospice provides spiritual support to patients, loved ones and fellow caregivers. The chaplain provides spiritual care interventions for patients and their loved ones to alleviate suffering. The chaplain represents the value of spiritual care in the overall care of our patients and is a resource to area religious organizations. The chaplain facilitates the involvement of religious leaders in their care of their members when they are receiving medical care from Mosaic Life Care Hospice. Chaplaincy services are always accessible throughout Mosaic Life Care Hospice.
  • This position is employed by Mosaic Health System.
Duties
  • Provides spiritual care to patients and their loved ones as an integrated clinical team member and engages in the development and initiation of the patient’s plan of care and documents their interventions in the medical record.
  • Engages specific duties related to providing palliative care, end of life care support, and bereavement care.
  • Responds to urgent patient care needs for support.
  • Responds to Advance Care planning needs for completing the documents and providing education and facilitates the process of completing the documents. Advanced care planning documents include but are not limited to: Out of Hospital Do Not Resuscitate (OHDNR) and Advanced Directives.
  • Offers support to fellow caregivers in their care of the patient and loved ones.
  • Other duties as assigned
Qualifications
  • Bachelor's Degree or Seminary degree or equivalent from an accredited institution required. Successful completion of one unit of Clinical Pastoral Education required. 4 units of CPE preferred.
  • Ecclesiastical endorsement or a letter of recommendation from ecclesiastical leader within 90 days is required.
  • CPR certification and valid driver's license are required.
  • Ordination or commission to function in a ministry of pastoral care is preferred.
  • Certification by a professional Chaplaincy certifying body is preferred.
  • 1 year pastoral experience in a congregational setting preferred.
