Details Community Caregiver - Serious Illness Management Serious Illness Management Full Time Status Day Shift Pay: $15.25 - $18.30 / hour Summary This position is responsible for assisting persons with advanced illness receive health care needs and/or accessing health care services consistent with established person-centered care plans. This position supports the interdisciplinary team to support the person to remain living in their community. This position is the frontline caregiver who works under the direction of the AIM RN Care Manager/Medical Social Worker and is employed by Mosaic Health System. Duties Supports the implementation of the person-centered care plan through provision of services; use of self-management skills; teach back techniques; coaching and recognition and reporting of red flag identifiers for specific health care conditions. Works with community agencies to assure appropriate communication/referral, and works as liaison between hospital and community agencies. The CC also facilitates follow up appointments and transportation Collaborates with interdisciplinary team, patient, and families, as well as participating in interdisciplinary team care planning conferences. The Community Caregiver will also support patients while they prepare to self-manage and escalate medical and social service issues to nursing and social work. Assist in the coordination of health-related services to increase member’s access to care and aids member in adhering to scheduled appointments consistent with member’s care plan. Provides member/family education about the member’s unique needs and risks, including financial and coping issues. The Community Caregiver will have knowledge of community resources for health and health-related services, and provide options to the patient. Follow regulatory and compliance standards and documents all care provided. Drive to patient homes as needed to provide services or ensure services are obtained (physician appointments, prescriptions, arranging or providing light housekeeping and bathing; trips to grocery store, healthy meal prep, visits center on patient goal, safety awareness, and patient red flag are reviewed, linkage to community resources, etc.) Adhere to compliance standards as set forth by regulatory agencies. Performs other duties as assigned. Qualifications High School Diploma. Healthcare degree and or certification is preferred. In lieu of a degree/certification the following qualification and experience may be accepted: minimum 2 years’ experience in health care related field. Heart Saver certification required within 90 days of start date. If operating a motor vehicle for business purposes, a valid driver’s license and insurance is required. Experience in healthcare, managed care, hospital or clinical setting with a minimum of two year experience based on educational background.
