A nurse navigator is a professional registered nurse with department-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Utilizing the nursing process, a nurse navigator provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of their care.

Duties

Completes patient assessment, identifies specific patient needs, and addresses barriers to care
Provides education and resources to patients and their family
Coordinates appointments and referrals
Facilitates shared decision making
Promotes advanced care planning
Develops relationships with referring physicians and ensures follow-up and treatment plan is communicated
Other duties as assigned

Qualifications

All required education is a minimum requirement. Higher levels of education are acceptable. Graduate of a school of nursing required.
RN - Registered Nurse - State Licensure And/Or Compact State Licensure, State Licensure And/Or Compact State Licensure in state, depending upon designated work location required and BCLS- Basic Life Support required within 90 days of hire.
2 years experience hospital or clinic nursing experience required. Previous Oncology and/or Navigator experience preferred.
