Summary
  • The LTACH Clinical Liaison is responsible for transitioning patients from the acute care environment to Mosaic LTACH, St. Joseph. The Clinical Liaison will advocate for referred patients in the pre-admission process by collaborating with discharge planners to assess patient needs. The CL will serve as a resource for healthcare professionals through fostering relationships within medical facilities. With clinical expertise and problem-solving skills, the CL will coordinate care plans with physicians and families. Additionally, the CL will assist in the facilitation of patient transfers and address barriers to admission. The CL will develop and maintain new and current relationships with various areas of Mosaic Life Care Hospital, Clinics and PAC providers. This position reports to the LTACH President. This position is employed by Mosaic Life Care.
Duties
  • Assesses potential patient's condition and past medical history and obtains appropriate documentation to support the appropriate post-acute care eligibility.
  • Knowledgeable of payor coverage requirements pertaining to LTACH criteria.
  • Provides an overview of the appropriate post-acute care service.
  • Communicates with hospital staff to establish a coordinated plan of care that reflects the appropriate post-acute care service to meet the patient's needs.
  • Follows all applicable federal and state regulations, including HIPAA, compliance and employment regulations. Complies with all Care Management and Mosaic Life Care policies and procedures.
Qualifications
  • Graduate from school of nursing required. BSN is preferred.
  • Nursing\RN - Registered Nurse - State Licensure And/Or Compact State Licensure in state, depending upon designated work location within 90 days is required. Clinical\BCLS - Basic Life Support required within 90 days. Clinical\ACLS - Advanced Cardiac Life Support - IF working in an inpatient area in which ACLS is required.
  • 3 Years of clinical experience in post-acute care required.
