Summary
The Care at Home at Mosaic Life Care in St. Joseph, MO is seeking to hire an additional Nurse Practitioner or Physician Assistant to join our cooperative, organized, and friendly practice in St. Joseph, MO. This position offers the security of joining an established health system with the lifestyle of practicing medicine in a comfortable and close-knit community. Providing care across multiple nursing facilities, focusing on Skilled Nursing Facility (SNF) and Long-Term Care (LTC) patients. Includes potential in-home visits. Primarily serving an elderly population, with an emphasis on delivering palliative and focused care to improve quality of life and help prevent hospital readmissions.

The team currently provides outreach services at nursing facilities within 50 miles of St Joseph Mosaic Campus. The current Care at Home team consists of three physicians, three advanced practice providers, and one nurse.

  • APP Requirements:
  • Successful completion of an APRN master’s nursing degree or graduate of accredited PA program
  • Current Missouri license preferred or the ability to obtain licensure
  • Current DEA licensure

Additional Qualifications:

  • Possess ability to work independently/autonomously and interdependently with other health care professionals
  • Have extensive patient assessment skills using diagnostic methods and instruments with the ability to develop appropriate care plans, modifying as indicated
  • Possess effective communication skills and interpersonal skills with patients, families and members of the health care team
Qualifications
  • Successful completion of a CNS or APRN master’s degree in a nursing program in the specialty area or must demonstrate successful completion of a formal post-graduate track or accredited program (within the specialty area) within a school of nursing granted graduate-level academic credit (e.g. graduate, non-matriculating program)
  • Licensed in state of Missouri or Kansas, depending upon designated work location. Must be currently certified by a nationally recognized certifying body acceptable to the Missouri or Kansas State Board of Nursing as well as hold and maintain a current Document of Recognition
