Summary
  • This position provides clinical oversight and coordination for LTACH wound care services. Accepts responsibility to assess, collaborate in patient care planning, implementation, evaluation and plan modification as well as supervise care for patients at risk for or have wounds. Work collaboratively with Providers and direct patient caregivers to deliver wound management that meets standards of wound care. Collaborates with leadership and caregivers to assess, plan, develop, implement, facilitate, evaluate and revise educational programming for all nursing personnel and non-nursing personnel as indicated or requested and implements a comprehensive education program. Educational programs typically include, but are not limited to orientation, continuing education and in-services. Other services may include, but are not limited to process, performance and service-delivery consultation and contributions to the organization through ad-hoc or permanently established councils, committees and workgroups. Meets the educational and informational needs of new caregivers, experienced caregivers and students. Participates in the operational leadership, focusing on new caregivers and the routine education needs of staff, and assists in improvement initiatives to utilize evidence-based practices to provide safe, high quality care for patients and families.
  • This position works under the supervision of the LTACH Director of Nursing and is employed by Mosaic Health Systems.
Duties
  • Wound Care: Provides wound care evaluations and document in the electronic medical record. Analyzes the information collected to determine patient needs and expected outcomes.
  • Assists nursing caregivers with development of plan of care for wound patients as well as for patients at high risk for wounds. Engages in caregiver, patient, and family teaching to achieve desired patient outcomes for wound management & prevention.
  • Works collaboratively with Providers to provide orders for wound care within national standards of care. Accompanies Providers on rounds.
  • Functions as a resource to staff for appropriate utilization of wound care dressings, supplies and pressure-reduction specialty beds and all wound care/wound prevention modalities.
  • Oversees ordering of wound care supplies, specialty beds and equipment. Monitors vendor billing for accuracy.
  • Other duties as assigned
Qualifications
  • All required education is a minimum requirement. Higher levels of education are acceptable. Bachelor's Degree in Nursing required and MSN, required to obtain within 4 years of hire/transfer is required.
  • RN - Registered Nurse - State Licensure And/Or Compact State Licensure in the State of Missouri is required. CWCN - Certified Wound Care Nurse or equivalent (CWS,WCC) within 3 years of start/hire is required. BCLS - Basic Life Support is required within 90 days of hire. ACLS - Advanced Cardiac Life Support within 90 days is required.
  • 3 years Clinical nursing experience and experience in wound care management required. 1 year Critical care nursing experience and experience in development and implementation of educational programs preferred.
