- This position evaluates, plans, & implements treatment programs for the rehabilitation of patient, as directed by a physician, using approved treatment procedures. Provide patient /family education. Participates in family conferences. Responsible for all necessary documentation.
- This position works under the supervision of the Associate Director or Lead RN and is employed by Mosaic Health System.
- Provides physical therapy services to patients according to a written physician’s plan of care.
- Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
- May train patient in the use of prosthetic device.
- Identifies patient and family/caregiver needs for other home health services and refers as necessary.
- Other duties as assigned.
- Graduate of accredited physical therapy program.
- Licensed Physical Therapist in State of Missouri within 60 days of hire required.
- BCLS - Basic Life Support certification within 90 days of hire required.
- Valid Driver's License
- 1 year appropriate experience as a physical therapist required. 1 year community/home health experience preferred.
