Summary
  • This position is responsible for the implementation of standards of care for medical social work services, assisting with care management in multiple settings, and provides clinical social work interventions, discharge planning and social services to Home Health patients. Accepts assignments to provide services in the patient's home. Provides exceptional compassionate service to all patients and customers. Is responsible for all necessary documentation as required by accepted medical guidelines and provides oversite of medical social workers documentation and care planning.
  • This position works under the supervision of the Lead Registered Nurse and is employed by Mosaic Health Systems.
Duties
  • Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the case manager. Carries out social evaluations and plans intervention based on evaluation findings.
  • Maintains clinical records on all patients referred to social work. Adhering to the Organization's documentation and care procedures and standards of personal and professional conduct. Provides accurate and timely documentation consistent with the plan of care.
  • Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
  • Provides information to patients or families/caregivers and community agencies. Serves as liaison between patients or families/caregivers and community agencies.
  • Maintains collaborative relationships with Organization personnel to support patient care. Participates in the development of the total plan of care and case conferences as required.
  • Assists patients to navigate benefit structure and financial resources.
  • Other duties as assigned
Qualifications
  • All required education is a minimum requirement. Higher levels of education are acceptable. Master's Degree in Social Work required.
  • Current certification or licenses by state law within the practicing state is preferred. BLS certification required within 90 days of start date.
  • Two (2) years experience in an acute care, nursing home, skilled nursing facility or home care setting required.
